Forms are online at the following site. Please select the link below and follow the directions to register on DragonFly. The physical exam form is available at the bottom of the page for you to print and take to your doctor. After your doctor visit, you should upload the form into DragonFly account.
Physical Exam Form
Physical exams MUST be dated AFTER May 1, 2023 for the 2023-2024 school year! Forms dated before May 1 are not valid for the 23-24 school year.
